Project Description: Landscape services: Weedeating down to bare earth around 7 greenhouses in order for plumber to have access to irrigation lines/valves; site review required before bid will be accepted.

Special Instructions
1) Vendor must provide a certificate of insurance naming University of Hawaii, Komohana Research and Extension Center, 875 Komohana Street, Hilo, HI 96720 as additional insured for General Liability and Auto with a waiver of subrogation in favor of UH for ALL policies listed (including Workers Compensation) in the amounts as shown on the attached example. A statement in the description box of \"Insurance liability is primary and non- contributory\" is mandatory. If vendor is a sole proprietor with no employees, Workers Compensation is not required, and vendor must provide a statement that there are no employees or contract hires that will be used to perform work on UH property. The COI must be provided before a purchase order can be issued for the vendor awarded this RFQ. See Example of COI attached to this RFQ.
2) Sign, date and return attached Office of Risk Management (ORM) Insurance Agreement with your quote. If you do not wish to sign the ORM, please email Debi Lowe (djbrockm@hawaii.edu) stating that you refuse to sign the document
3) If quote is $2500 or more, vendor must be compliant with the Hawaii State Tax Department at https://vendors.ehawaii.gov/hce/
4) Contact Debi Lowe (808-990-5887) to coordinate a site visit to assess the project
5) Vendors who have not done business with the University of Hawaii, or have not done business with the university after July 1, 2012 will be required to submit a 2024 W-9 tax document
6) Vendors must agree to the University of Hawaii\'s terms and conditions in place of Vendor’s own terms and conditions. UH Terms and Conditions attached.
7) An acceptable quotation must conform to this Request for Quotation in all material respects. Any of the following may be grounds for disqualification:
(a) Take exception to any of the specifications, terms and conditions in the RFQ
(b) Place conditions on the furnishing of solicited good and/or services
(c) Inclusion of a quotation or order form containing additional specifications, terms or conditions
(d) If any negotiating is requested in regard to this RFQ, not responding within five (5) business days at any point during the negotiation process (before issuance of a purchase order)
(e) Failure to follow directions and instructions in the RFQ
8) The vendor who is awarded the RFQ must submit all requested documentation, including the signed and dated award within five (7) business days of the award notification
