Special Instructions
Basis for award: The solicitation includes SEVEN (7) items. The award of purchase orders shall be made to the responsible offeror submitting the lowest quote for each individual line item. The award is subject to availability of funds. Offerors are not required to submit a quote for all items listed in order to be eligible for an award. Awards may be issued to multiple offerors based on the lowest price offered for each line item. Partial awards will be considered. Quotes submitted for each individual line item listed shall include all costs required to furnish the specified line item, to include furniture item cost, assembly, installation services, shipping, freight, and all applicable fees and taxes for that particular line item.

All deliveries will be to: Department of Public Health Sciences, 1960 East West Road, Biomedical Sciences, Honolulu, HI 96822. Specific room numbers will be disclosed upon award.

Quotes are only accepted from the manufacturer or authorized resellers.

Quotes must include tax (including the Hawaii General Excise Tax of 4.712%) and shipping fees, freight, and all applicable charges. If taxes and shipping fees are not listed, it will be determined that taxes and shipping fees are included in the bid price.

NO SUBSTITUTES unless prior approval by buyer.

If bid exceeds $2,500.00, vendor must furnish proof of compliance with the requirements by presenting a Certificate of Vendor Compliance (Hawaii Compliance Express: https://vendors.ehawaii.gov). Awards cannot be made to non-compliant vendors.

If you are compliant but have not done business for the University of Hawaii you may be required to submit a W-9 tax document per University policy.

If the vendor is not compliant within TEN (10) days or what is mutually agreeable after notification of intent to award by the University, the vendor\'s quotation will be disqualified.

ALL furniture must be delivered fully assembled.
