Project Description: Labor and materials to troubleshoot 6 inoperable overhead doors/fire curtains (10-12’ height)
in the Medical Education Building at the John A. Burns School of Medicine, 651 Ilalo Street.
Rolling Steel Fire Doors are Overhead Doors and Face Mounted Series 630 LFN/LFE.
The ceiling-mounted controllers per door are also Overhead Doors Fire Sentinel fail-safe units.
Work is for troubleshooting only, no repairs unless they can be performed at no additional
cost. Troubleshooting findings shall be detailed in writing to the University and repair work
will be addressed under separate procurement.

Bid price shall be valid for a minimum of 45 days.
Vendors who attach additional documents (quotations, proposals, etc.) with terms and
conditions shall be disqualified.
Under Hawaii law, HRS 103D-310(c), you must provide proof of compliance in order to
receive a contract worth $2,500 or more with state and county government entities in
Hawaii.
Vendors need to obtain a copy of their “Certificate of Vendor Compliance” via Hawaii
Compliance Express. To register with Hawaii Compliance Express, go to
https://vendors.ehawaii.gov/hce/splash/welcome.html. Vendor must provide proof of
compliance 1 day after closing date.
