Project Description:
Provide monthly maintenance and a required hydraulic weight test on four (4) elevators and monthly maintenance for one (1) chair lift according to the terms listed in Attachment A. The designation and location are as follows:

Special Instructions
1. Any job site evaluation shall be by appointment only and completed by 12 pm on September 2, 2026. Please email Tami Langohr at langohr@hawaii.edu by 10 am on September 1, 2026 to schedule your visitation. She will provide confirmation no later than 12 pm on September 1, 2026.

2. The one-year maintenance will include monthly maintenance and hydraulic weight testing; the exact starting date of service shall be coordinated with Kauai CC\'s Facility Manager and/or Vice Chancellor for Administrative Services after issuance of the purchase order. 3. All quotes shall be valid for 90 days after the closing date of the RFQ, and only aggregated bids shall be considered for award.

4. Upon award, the awardee must provide a Certificate of Insurance meeting the insurance requirements as specified within the Special Conditions (Attachment B), and a Certificate of Vendor Compliance within three (3) business days.

5. The University reserves the right to determine quality workmanship and response time to each occurrence. A response time within 4 hours from the reported trouble call is expected. All elevators in this contract cannot be inoperative for more than 2 days, excluding performing a major overhaul to an elevator.

6. Campus business hours for this contract are 7:45 am to 4:30 pm, excluding weekends and State-recognized holidays.

7. Quotes shall include all labor, materials, supplies, equipment, air travel, ground transportation, rentals, scheduling and procuring the State elevator inspector, all fees, and applicable taxes. As in past practice, the campus will only be obligated to pay for service fees for the permit to operate that are invoiced by the Elevator Inspection, Department of Labor and Industrial Relations, Hawaii Occupational Safety & Health Division, Boiler Elevator Inspection Branch, 830 Punchbowl Street, Honolulu 96813.

8. Any discrepancies found by the State elevator inspector shall be corrected to their satisfaction and are part of this proposal for quotations.

9. A contractor\'s license per Hawaii Revised Statutes Chapter 444 is required.

10. Should the offeror require any of the following, their bid may be disqualified:
(A) Taking exception to any of the specifications, terms, or conditions contained in the RFQ;
(B) Placing conditions on the furnishing of items offered;
(C) Inclusion of a quotation or order form containing additional specifications, terms, or conditions;
(D) Referencing external documents containing additional specifications, terms, or conditions;
(E) The University reserves the right to any possible exemption based on technical merits or pre-existing conditions.

11. Under section 103D-310 Hawaii Revised Statutes, all offerors upon award of contract, must ensure compliance with all laws governing entities doing business in the State of Hawaii. Offerors will be expected to provide their “Certificate of Vendor Compliance” identifying their current status as “Compliant” on Hawaii Compliance Express: http://vendors.ehawaii.gov/hce/ once a contract is awarded.

12. All awards are subject to the University of Hawaii’s Terms and Conditions applicable to all Purchase Orders; failure to agree and/or submission of separate terms and conditions may warrant disqualification (See Attachment C).
