Project Description:
Scope of Work

1. Furnish and install approximately 5,877 SF of commercial-grade modular carpet tile suitable for high-traffic institutional/event use, Pilina Event Center (PEC) and two offices.

2. Provide carpet tile adhesive and all materials required for installation.

3. Remove existing VCT wax and perform necessary patching and leveling to prepare the floor for carpet installation.

4. Furnish and install approximately 4-inch rubber cove base, including required adhesive.

5. Include all installation labor, equipment, supervision, and materials.

6. Include mainland-to-Maui freight/delivery, unloading, and handling.

7. Include removal and disposal of existing flooring, debris, and installation waste.

8. Complete final cleanup and leave the area ready for use. All costs associated with final cleanup shall be included in the applicable pricing.

9. Vendor shall identify the proposed carpet manufacturer, style/product, color, and warranty with the quotation.

Special Instructions

General Instructions and Requirements:

1. Bidders are advised to carefully and thoroughly review all Special Instructions and Specifications prior to submitting a bid.

2. Bids may be disqualified if incomplete or if required information is not provided.

3. Vendor must submit a detailed quote showing pricing and specifications for each item identified in this RFQ.

4. Vendor must be capable of providing the entire order as specified in this RFQ.

5. Quotes must include all applicable handling, shipping, delivery, freight, and other fees, as well as applicable taxes. The applicable tax rate shall not exceed 4.7120% for Maui County.

6. If a bid is submitted based on a substitute or equivalent product, the vendor must provide detailed specifications for the proposed product. RCUH reserves the right to determine equivalency.

7. Quotes must remain valid for 60 days after the bid closing date. The quote must clearly indicate the validity period.

8. Applicable terms and conditions must be submitted with the bid.

9. Buyer’s acceptance of an offer is conditional upon the Seller\'s acceptance of the Buyer\'s terms and conditions. See attached RCUH Terms and Conditions, Attachment 31 and Attachment 32a.

10. Purchase Orders will be issued by the Research Corporation of the University of Hawai’i (RCUH).

11. Preferred payment terms are Net 30.

12. Quantities are subject to change based on funding availability.

13. The awarded vendor must provide a current, signed W-9 upon request.

14. The awarded vendor must have a valid State of Hawai’i Tax Clearance (Certificate of Vendor Compliance), as applicable to purchases and final payments of $25,000 or more.

15. Attachment 22 - Certification Regarding Debarment, Suspension, and Other Responsibility Matters: Offeror shall complete, sign, and submit Attachment 22 with its quotation.

16. Certificate of Insurance (COI): Offeror shall submit a current Certificate of Insurance (COI) meeting University of Hawai’i requirements with its quotation.

17. All materials and installation must be completed by September 30, 2026, at UH Maui College, 310 W. Kaahumanu Ave., Kahului, HI 96732.

18. Vendor shall coordinate delivery and installation with UH Maui College.
