Project Description:
Existing stainless steel refrigerator/freezer to be removed from property: The new
refrigerator/freezer unit is not to exceed 83” tall x 30” wide x 32” deep and be NSF-
certified for small commercial kitchen used for demonstration projects for University
of Hawaii, College of Tropical Agriculture. The refrigerator section must maintain
temperature of 41 degrees F or lower and the freezer section must maintain temperature of 0
degrees F or lower, preferably below 6 Amps, but no more than 6 Amps, 115V, PH 1.
UH prefers freezer section to be on the bottom, if available. Vendor can include more
than one unit on the bid, knowing UH will chose only 1 unit; pricing must be inclusive of
tax.
Special Instructions
1) Quotes of $2500 or more require UH to download a Hawaii tax compliance certificate from
the state website https://vendors.ehawaii.gov/hce/ for all vendors. Bids from vendors not
in compliance shall be rejected if vendor does not become compliant within two days after
UH advises of a bid award
2) Vendors who have not done business with the University of Hawaii, or have not done
business with the university after July 1, 2012 will be required to submit a 2024 W-9 tax
document
3) Vendors must agree to the University of Hawaii’s terms and conditions (attached) in
place of Vendor\\\\\\\'s own terms and conditions.
4) Vendor must have minimum liability insurance for general, auto and workers compensation
as specified in the attached example, including the University of Hawaii to be named as
additional insured for general and auto with waiver of subrogation for all policies listed
and a statement in the description box that liability insurance is primary and non-
contributory where required by written contract. If vendor consigns another company for
removal and set up of the unit, the other company must provide the COI per the attached example
coordinated by the vendor awarded this bid
5) Quote must include cost to remove old refrigerator/freezer unit and haul away, include
installation of new unit, ensure the new unit operates quietly and each section cools to temperatures
required by HDOH for commercial kitchens. See attached pictures of current set up and electrical plan for
kitchen.
6) Vendor will provide operating instruction at time of delivery and will provide a
physical or digital manual for the care and operation of the unit
7) Award of this RFQ is contingent on funding if all bids are over the current funding
already set aside based on market research, contractor labor costs, and shipping. It has
been assumed after the old unit is removed, installing the new unit is a plug and play.
8) The invoice MUST be emailed within three days of installation and proof of operational
integrity of the unit to Debi Lowe, djbrockm@hawaii.edu
9) An acceptable quotation must conform to this Request for Quotation in all material
respects. Any of the following may be grounds for disqualification:
(a) Take exception to any of the specifications, terms and conditions in the RFQ
(b) Place conditions on the furnishing of solicited good and/or services
(c) Inclusion of a quotation or order form containing additional specifications, terms or
conditions
(d) If any negotiating is requested in regard to this RFQ, not responding within two (2)
business days at any point during the negotiation process (before issuance of a purchase
order)
(e) Failure to follow directions and instructions in the RFQ
