Project Description:
This project is to replace stair finish in the George Hall. Scope of work includes demolition of existing stair finish, installation of new resilient stair tread, hazardous material abatement, and miscellaneous related work.

Special Instructions
1. All offerors shall review the attached documents before submitting a quotation.

2. Attached to this RFQ are the following documents:

a. Mandatory Terms and Conditions which includes the Notice to Offerors, Quotation Information, Special Provisions, and General Provisions.

b. Drawings and Technical Specifications.

3. A site inspection will be held and will be only opportunity to review the areas of work, refer to attached Notice to Offerors.

4. In the event that the offeror is awarded this RFQ and its remittance address differs from the offeror\\\'s order address, indicate the remittance address on the Note to Buyer.

5. Offer pricing shall be held for minimum 90 days.

6. Offeror to submit the following upon receipt of Superquote Award notice:
-W-9
-Hawaii Compliance Express “Certificate of Vendor Compliance”
-Certificate of Insurance and any other insurance requirement
-Listing of Joint Contractor and/or Subcontractors OPRPM Form 54
-Copy of Valid State of Hawaii Contractor\\\'s License B or C-21
-Certification of Compliance with HRS 396-18, Safety and Health Programs (for proposals in excess of $100,000)
(Documents shall be scanned and emailed to TRPO as a pdf file)
Failure to submit these documents to the TRPO within 10 working days of award notification sent by TRPO to the awarded Offeror may result in selection of next lowest bidder.
