Project Description:
Scope of Work:
The project consists of the demolition and reconstruction of two (2) damaged concrete reinforced concrete bridge railing sections in accordance with the solicitation and project plans/specifications attached.

Bid Instruction:
Contractor shall read bidding instructions prior to bid. Failure to follow these instructions may result in rejection of the bid. State reserves the right to remove Line Item No. 1 to reduce the total bid cost. If Line Item No. 1 is removed, the State will provide the development of traffic control plans and implementation of traffic control.

1. Use Line Item No. 1 to bid solely for the development of traffic control plans and implementation of traffic control. Associated costs shall include, but are not limited to the design services, labor, equipment, materials, signage, all applicable taxes and incidental work.
2. Use other line items to bid for each of its respective work noted in Line Item descriptions.

General Notes:
1) Methods will be evaluated and selected based on the best interest for the state.
2) The Contractor shall provide the materials and tools and shall be available to being work immediately.
3) All work shall be in accordance with the State of Hawaii 2005 Standard Specifications for Road and Bridge Construction and Special Provisions prepared for this project.
4) Contractor shall field verify all existing conditions prior to bid. Notify Engineer if any discrepancies are found.
5) Contractor shall provide necessary best management practices (BMP) for storm water pollution including permits and devices. Contractors shall prevent any debris/dust from entering streams/waters.
6) Contractor shall secure/maintain work site. No concrete/metal can be left on site.
7)Contractor must dispose of all debris at an authorized deferral, state, and/or city location.
8) Contractor must be highly skilled in this type of repair with proof of and in good standing.
9) Work may be performed only between the hours of 7:00 am and 2:30 pm, Monday through Friday, except State Holidays, unless otherwise approved in writing by the District Engineer.
10) Contractor shall contain licenses of type A or types C-31a, C-41, C-13
11) If a bidding Contractor intends to use one or more subcontractors to perform any portion of the work under this contract, the bidder must identify each subcontractor in its bid submission. The bid must include the name of each proposed subcontractor and the portion of the work to be performed by that subcontractor. Failure to disclose a subcontractor as required may result in rejection of bid, or being deemed nonresponsive, subject to applicable law and the terms of the solicitation.
12) Contractor shall secure work at the end of each work shift to provide safe access for traffic.
13) Contractor shall provide 2 extra days of traffic control for pre-final and final inspections

Cancellation: This solicitation may be cancelled and any or all bids rejected in whole or in part, without liability to the State, when it is determined to be in the best interest of the State.

Bid price must be the total price including taxes, permits, BMP, etc.

Bond Requirements: All bidders shall be required to include a Performance Bond and a Payment Bond in accordance with HRS §103D-324 upon award of the contract. These bonds must be equal to 100% of the contract price.

GENERAL NOTES FOR TRAFFIC CONTROL PLAN:
1. Contractor shall submit the site-specific traffic control plans intended for use during construction for review and approval of the Engineer prior to start of work.
2. Include a signed certification statement affirming that the site-specific Traffic Control Plans submitted to HDOT has been prepared under the direction or supervision of a Licensed Professional Engineer, is in compliance with the current HDOT, MUTCD, and FHWA standards, and is an appropriate application of traffic control measures for the construction work to be performed.
3. Contractor shall submit requests for detours and lane closures to the Engineer. Advanced public notification is required.
5. All traffic lanes shall be a minimum of 10’ wide.
6. Contractor shall be responsible for all lane closure safety.
