Maui College Library Materials Relocation Project
Scope of Services
Moving services are required to relocate approximately 256 Boxes of books,
periodicals,
archives, and other sensitive and fragile library materials, along will 17 tables:
* Origin: Ka Lama Building, Room 108
* Destination: Library Building, Main Floor (designated as the \\\\\\\"2nd Floor\\\\\\\" in
Attachment 6)
* Location: University of Hawaii Maui College, 310 W. Ka\\\\\\\'ahumanu Ave., Kahului HI

The vendor shall provide all labor, supervision, transportation, equipment, packing
materials, and related services necessary to complete the relocation safely and
efficiently.
Phase I - Building-to-Building Relocation Services
The selected vendor shall:
* Relocate approximately 256 boxes of library materials and 17 tables to the
Library Building Main Floor or nearby discard area, as directed by Library staff.
* The Beige Cabinet in Ka Lama room 107 returns to Ka Lama room 108 as directed by
Library staff.
* Use vendor-provided equipment and vehicles suitable for transporting sensitive
and fragile library materials.
* Protect all materials during handling and transit, including protection from
weather conditions such as rain and high winds.
* Repack items, if necessary, to ensure safe and secure transport.
* Supply all required packing materials, including but not limited to:
○ Durable moving boxes
○ Packing paper
○ Tape
○ Protective wrapping materials
* Arrange and place boxes in the exact order and designated locations identified in
the Library Main (2nd) Floor Diagram (Attachment 6).
* Exercise care to maintain the organization and integrity of all library materials
throughout the move.
* Remove and properly dispose of all packing materials and debris upon
completion of the relocation.
Phase II - Internal Relocation Services
* Anticipated timeframe for relocation of the following shall be subject to
final confirmation by UH Maui College Library representative, Shannon Alueta:
○ 256 boxes from the Library Main Floor (2nd Floor) to the 3rd Floor via
stairs only; elevator is unavailable
○ Repack items, if necessary, to ensure safe and secure transport.
○ Supply all required packing materials, including but not limited to:
* Durable moving boxes
* Packing paper
* Tape
* Protective wrapping materials
○ 6 of the 17 tables from the Library Main Floor (2nd Floor) to the 3rd
Floor via stairs, elevator is unavailable.
○ 11 of the 17 tables will remain on the Library Main Floor (2nd Floor) but
will be placed in accordance with the Phase II: Draft Diagram of UHMC
Library 2nd & 3rd Floor.
* Project completion estimated date is subject to final confirmation and
scheduling coordination with Shannon Alueta.

to ensure boxes and tables are placed in the exact same sequence and
arrangement upon arrival during Phase I.
* Attachment 7: “Phase II: Draft Diagram of UHMC Library 2nd & 3rd floor” is
provided to ensure boxes and tables are placed appropriately during Phase II.
* Vendor shall coordinate scheduling and access requirements with UH Maui
College representatives prior to the move date.
Special Instructions
Vendors are advised to review all the special instructions and specifications
carefully and thoroughly.
1. The submittal of a quote shall be considered as acceptance of the
specifications as published, as well as agreement to the University
of Hawaii Terms and Conditions, Applicable to All Purchase Orders.

2. Hawaii Compliance Express: Quotes of $2,500 or more will require
a Certificate of Vendor Compliance in compliant status before purchase
order is issued (HRS 103D-310(c) as the University of Hawaii must verify
compliance with all laws governing entities doing business in the
State of Hawaii. If a vendor does not have a compliant certificate,
the vendor\\\\\\\'s quote will be disqualified, and the next lowest quote
will be considered. For more information, refer to

3. Award will be in aggregate and subject to availability of funds. The award of
contract, if awarded, shall be made to the lowest responsive and responsible
bidder on the Total Price. The University of Hawaii reserves the
right to cancel the Request For Quotation due to funding.
4. Quote must be valid for a minimum of 60 consecutive calendar
days after the bid closing date.
5. Quote Price shall be F.O.B destination, including all material
costs, labor costs, equipment costs, transportation costs,
vehicle costs, shipping costs, supply costs, insurance premiums,
rental costs and all applicable taxes.
6. Tax Liability: Both out-of-state and Hawaii bidders are advised the
amount bid on this Request for Quotation is subject to General Excise
Tax (GET) imposed by Chapter 237, Hawaii Revised Statutes (HRS).
Maximum Tax Rate for Maui County is 4.712%. Tax shall be considered
included in your total cost quoted.
7. Phase1: Delivery is required within 30 days after receipt of order (ARO).
Do not schedule or make any purchases until the approved purchase
order is provided.
Phase II: tentative completion 120 days after receipt of the purchase
order (ARO).
8. Damages incurred during shipment, assembly and/or delivery
of goods/products will be the responsibility of the vendor.
9. Upon request the following shall be provided to the University:
a) Itemized quote
b) Information on any past experience with similar projects
10. Site Visit: A sit visit is highly recommended.
