Project Description:
Recoat Hale ʻImiloa Veterinary Technology Facility existing epoxy flooring system at the Windward Community College per the attached technical specifications.

Special Instructions
1. All bidders shall review the attached documents before submitting a quotation.

2. Quote to include all labor, materials, tools and equipment necessary to complete the project in accordance with the attached documents.

3. All interested parties may view the site. Please contact Andy Li at 808-235-7369 or via e-mail at andy.li@hawaii.edu to schdule a visit prior to July 3, 2026.

4. Quote must be valid for 90 days. Project must be completed during the period August 3 to 14, 2026.

5. Vendor’s current status MUST be listed as “COMPLIANT” in Hawaii Compliance Express by the closing date of the bid and through the issuance of the purchase order.

6. To be eligible to bid, bidder must possess a valid state of Hawaii Contractor\\\'s license C-55 or license B.

7. Attached to the RFQ are the following documents:

(2) Vet Tech Facility Floor Plan
(3) Attachment A-Insurance-Agreement
(4) COI Sample
(5) PO-Terms-Conditions
(6) Wage Certification

8. Vendor receiving the award is required to provide the following documentation before the purchase order is issued. Note: Do not purchase any materials until approved purchase order is provided.
(1) Hawaii Compliance Express Certificate of Vendor Compliance
(2) Certificate of Insurance (indicating University of Hawaii, Windward Community College as the Certificate Holder)
(3) Certificate of Compliance with HRS 396-18
(4) Signed Superquote Award Notice
