Special Instructions
Aloha,
The University of Hawaii Environmental Health & Safety Office requests a quotation for
hazardous waste disposal services of gas cylinders located at Watanabe Hall on the UH
Manoa campus.
The attached Technical Specifications and Insurance Requirements are the minimum
requirements for a bid. Please read them carefully. If a vendor has not performed waste
disposal services for the University of Hawaii before, samples of hazardous waste
manifests shall be provided, along with the bid, to demonstrate the ability to handle
waste disposals of similar complexity (i.e. Multiple DOT hazard classes). If the samples
are not submitted, the bid will not be accepted. If you plan to combine drums, be aware
that any combining must be in accordance with DOT and EPA regulations and is subject to
the approval by the University of Hawaii Hazardous Material Officer.
The quotation total must include all shipping and handling charges as well as any taxes
that may be assessed. No partial bids are allowed.
At least a week prior to waste pick-up, the manifests and lab pack inventories will have
to be submitted to Carolyn Oki-Idouchi at the University of Hawaii at Manoa Environmental
Health & Safety Office via email (coki@hawaii.edu) for review and comment.
PLEASE SUBMIT YOUR QUOTATION NO LATER THAN 2:00 PM on Tuesday, June 23, 2026. The
quotation must be signed by an authorized representative of your company. Quote must be
valid for 25 days after the award date.
The contracted company must provide service (i.e. remove all waste listed on the inventory
from University Property) by August 7, 2026.
Any inquiries shall be directed to Carolyn Oki-Idouchi at 808-956-3198, or email at
coki@hawaii.edu.
Mahalo,
Carolyn Oki-Idouchi
Hazardous Material Management Officer
