Project Description:
The project in general consists of interior improvements at Hawaii Hall room 307. The scope of work includes selective demolition, drywall, flooring, painting, electrical and miscellaneous related work.

Special Instructions
1. All offerors shall review the attached documents before submitting a quotation.
2. Attached to this RFQ are the following documents:
a. Mandatory Terms and Conditions which includes the Notice to Offerors, Quotation Information, Special Provisions, and General Provisions for Construction.
b. For awards in excess of $100,000: Certification of Compliance with HRS 396-18, Safety and Health Programs.
c. Drawings
3. A pre-bid conference will not be held. Offerors can access the site by contacting Matt Arakaki at mmarakak@hawaii.edu or (808)956-7202.
4. In the event that the offeror is awarded this RFQ and its remittance address differs from the offeror\'s order address, indicate the remittance address on the Note to Buyer.
5. Offer pricing shall be held for a minimum of 90 days.
6. Offeror to submit the following upon receipt of SuperQUOTE Award notice:
- IRS Form W-9
- Hawaii Compliance Express “Certificate of Vendor Compliance”
- Certificate of Insurance
- Listing of Joint Contractor and/or Subcontractors OPRPM Form 54.
- (for awards in excess of $100,000) Certification of Compliance with HRS 396-18, Safety and Health Programs
- Signed SuperQUOTE Award Notice (Documents shall be scnned and/or emailed to TRPO as a pdf file)
