Project Description:
Provide monthly inspection and maintenance service of two (2) Hurst Steam Boilers (Model S5-G-125-150) for a period of six (6) months.

Provide Monthly Chemical treatment (increased frequency if required) on both Steam Boilers for a period of six (6) months.

Quote must include all applicable labor costs, supplies, overhead, and all applicable city,state and federal taxes. Quote shall be considered firm and cannot be modified after RFQ end date.

Under Hawaii law, HRS 103D-310(c), you must provide proof of compliance in order to receive a contract worth $2,500 or more with state and county government entities in Hawaii. Vendors need to obtain a copy of their “Certificate of Vendor Compliance” via Hawaii Compliance Express. Delivery requirements is 45 days ARO (after receipt of order). Please cite any type of supply related issues that may delay delivery outside of the 45 day requirement and an exception may be applicable.
