Quote shall be FOB destination and must include any shipping/freight fees, and all applicable taxes.

SUBSTITUTION REQUESTS
Substitutes are subject to prior approval. Specifications Sheet of substitutes must be submitted to lna4@hawaii.edu by 3pm PST on 01/14/2026 or it will not be considered. We reserve the right to determine equivalency. Approved substitutes will be posted in an addendum by 3pm PST on 01/16/2026.

Receiving Hours: M-F 7:30am-11:30am AND 12:30pm-3:00pm

DELIVERY REQUIREMENT
Delivery 2-4 weeks upon receiving Purchase Order. Partial deliveries will be accepted, with full delivery completed by the end of the 2-4 week period. Please provide anticipated lead time in your bid.

INVOICING REQUIREMENT
Vendor invoice must include Customer Name, PO #, Lamp Model #, and Quantity of each lamp model.

QUOTE REQUIREMENT
Quote shall be FOB destination and must include all shipping/freight fees, all applicable taxes, and anticipated lead time. DO NOT INCLUDE HAWAII ENERGY REBATE. REBATE REQUEST WILL BE SUBMITTED BY THE UNIVERSITY.
