Project Description: To supply and replace the existing Simplex Grinnell Fire Alarm Control Panel in the Hookipa Building and all associated components, parts, material, and labor to have a fully functional Simplex Grinnell Fire Panel system. Installation of new Fire Alarm Control Panel (New Fire Alarm control/monitoring panel must be compatible with existing devices. Subject to prior approval if offering substitute items.) Program equipment to restore system to manufacturer\\\\\\\'s original standards. Perform testing to ensure proper operation. Removal and disposal of old unit and all associated parts. Work to be performed by authorized dealer/distributor for Simplex Grinnel fire systems with factory trained and authorized service/repair technicians. Fire system shall be in compliance with the current National Fire Protection Association (NFPA-72) Standards, Contractor’s Board Licensing, State and Local Fire regulations.

SYSTEM DESCRIPTION
Existing system: Simplex 4100
Location: Hookipa Building 2208
Condition: Device in “fault” position and cannot be repaired due to age and deterioration.

Price shall include all labor charges for termination, removal and disposal of the old system, and installation, programming and testing of new equipment, as well as the cost for materials, tools, equipment, shipping & handling, transportation, freight, travel expenses, and applicable taxes and fees.

IMPORTANT NOTES:
▪︎ Work may involve temporary shutdown of Hookipa Building (2208) electrical service.
▪︎ All work to be done during normal campus hours: Monday - Friday from 7:00 a.m. - 3:30 p.m. Hawaii Standard Time (HST).
▪︎ Must be completed, tested, and certified operational within 45 consecutive calendar days from the date designated in the approved Purchase Order.
▪︎ Vendor must properly dispose of project waste off UHMC campus property.

1. All vendors are advised to review the Special Instructions and all attachments to this Request for Quotation. Submission of a quote confirms that the vendor has carefully examined these documents, and agrees to the University of Hawaii Terms & Conditions applicable to all Purchase Orders.

2. QUOTE VALID FOR: Minimum of ninety (90) consecutive calendar days after the Request for Quotation closing date.

3. BASIS FOR AWARD: Award will be based on the total aggregate amount. Award is subject to availability of funds.

4. REQUIRED DOCUMENTATION FOR AWARD: Vendor receiving the award is required to provide the following documentation before purchase order is issued:

(1) Hawaii Compliance Express Certificate of Vendor Compliance
(2) Certification of Insurance (refer to Special Provisions Attachment 2)
(3) Contractor Licenses Required: C15a or C-15a
(4) Itemization of quote: a) Labor cost: Hourly rate (and labor classification) x total number of hours for a grand total labor cost, b) Material cost including manufacturer and model/product number
(5) Attachment C Bid Bonds/Performance Bonds (as applicable)
(6) Listing of Joint Contractor and/or Subcontractors
(7) Certification of Compliance with HRS 396-18, Safety and Health Programs (as applicable)
(8) Signed Super Quote Award notice

5. QUESTIONS & REQUESTS FOR SUBSTITUTION: shall be submitted via email to David Tamanaha, email davidt@hawaii.edu no later than 3:00 p.m. Hawaii Standard Time (HST), September 30, 2025.

6. SITE VISIT HIGHLY RECOMMENDED: A site visit will be held on September 26, 2025. Please meet David Tamanaha at the Operations & Maintenance Building at 9:00 a.m. The initial inspection is to identify and examine the conditions and to be aware or satisfied as to the character and amount of work to be performed as called for by the specifications. No additional allowance will be granted because of lack of knowledge of such conditions.
