Project Description:
1) Repair bridge girder per plans attached. See plans for description of work and location.
2) Contractor shall provide the materials and tools, and shall be available for work immediately.
3) All work will be in accordance with State of Hawaii 2005 Standard Specifications for Road and Bridge Construction and Special Provisions (if available) prepared for this project.
4) Contractor must be highly skilled in this type of repair with proof of and in good standing.
5) Contractor will provide necessary best management practices (BMP) for storm water pollution including permits and devices.
6) Contractor will prevent any debris/dust from entering any downstream waterways.
7) Contractor will field verify all existing conditions prior to bid. Notify Engineer if any discrepancies are found.
8) No concrete/metal can be left on site, tarp below all work in dry stream, maintain/secure work site.
9) Work can be performed only between the hours of 7:30 a.m. and 3:30 p.m., Monday through Friday, except State Holidays, unless otherwise approved in writing by the District Engineer.
10) Minimum one lane of traffic open during construction. Contractor may shift lanes and utilize shoulder as required. Contractor will also provide access to Aliinui Drive during construction.
11) Contractor will submit traffic control plans for review and approval prior to start of work. Traffic control costs will be included in contractor's bid. Advanced public notification for lane closure is required.
12) Contractor will be responsible for all lane closure safety.
Bid price must include all taxes and other costs (i.e. traffic control, BMP, permits, bonds, etc.).
Cancellation: This solicitation may be cancelled and any or all bids rejected in whole or in part, without liability to the State, when it is determined to be in the best interest of the State.
Bond Requirements: All bidders shall be required to include a Performance Bond and a Payment Bond in accordance with HRS §103D-324 upon award of the contract. These bonds must be equal to 100% of the contract price.
