Special Instructions
1. Furnish all labor, materials, tools and equipment necessary to complete all work as specified, as called for on the Technical Specifications & Quote Forms.

2. In addition, the quotation includes all material and non-material costs, labor costs, equipment costs, transportation costs, vehicle costs, air freight costs, shipping costs, supply costs, insurance premiums, rental costs and all applicable taxes, etc.

3. Quote shall be valid for a minimum of 60 days after the closing date.

4. License Requirement: Vendor must have a valid certificate from either a school or association that adheres to the National Fire Protection Association (NFPA-96). The vendor must follow all the codes and standards set forth by the National Fire Protection Association (NFPA-96).

5. The attached documents must be adhered and referred to throughout this process:

Attachment# 3 - Technical Specifications
Attachment# 4 - University of Hawaii Terms and Conditions

6. Site visit highly recommended. A site visit will be held on Wednesday, August 13, 2025. Please meet David Tamanaha, at the Operations and Maintenance Building at 10:00 a.m.

7. Submit the following documentation with your quote submission:
a) Quote Form Schedules A-D
The line item 1-4 prices on your quote must match the corresponding pricing schedules A-D. Ensure that the total of all schedule amounts match the Request for Quotation Total.

b) Proof of Certification from either school or association that adheres to NFPA-96 to your quote package.

8. Quotes of $2,500.00 or more will require a Certificate of Vendor Compliance in compliant status at time of award (HRS 103D-310(c). If a vendor does not have a compliant certificate, the vendor\'s quote will be disqualified.

9. Vendor receiving the award is required to provide the following documentation before the purchase order is issued.

(1) Certificate of Liability Insurance
(2) Signed SuperQuote Award Notice
(3) Hawaii Compliance Express - Compliant Certificate
