Project Description:
PROJECT: UH Maui College Electrical Upgrade - Kaaike Building Server Room 221

LOCATION: Kaaike Building 2252 Server Room 221

Provide labor and material to install the following equipment in accordance with the Code of Federal Regulations (CFR), and rules and regulations at all levels. Vendor should have the proper equipment to test equipment functionality after project completion.

PROJECT SCOPE:
Qty (2) Additional 110V 20A duplex receptacles installed in two separate locations.
Please reference attached photo in Attachment #01 Technical Specifications indicating the two outlet locations noted by “X”.
Receptacles will be under the raised floor under the respective server racks.

IMPORTANT NOTES:
▪︎ Work will involve temporary shutdown of Kaaike Building 2252 electrical service.
▪︎ ALL WORK TO BE DONE ON SUNDAY.
▪︎ Must be completed, tested, and certified operational within 120 consecutive calendar days from the date designated in the approved Purchase Order.
▪︎ Vendor must properly dispose of project waste off UHMC campus property.

1. All vendors are advised to review the Special Instructions and all attachments to this Request for Quotation. Submission of a quote confirms that the vendor has carefully examined these documents, and AGREES to the University of Hawaii Terms & Conditions applicable to all Purchase Orders.

2. CONTRACTOR LICENSE REQUIREMENT: To be eligible to submit a quote, vendors must possess a valid State of Hawaii “C13” License.

3. Quote shall include all labor charges, as well as the cost for materials, tools, equipment, shipping & handling, transportation, freight, travel expenses, and applicable taxes and fees.

4. BASIS FOR AWARD: Award will be based on the total aggregate amount. Award is subject to availability of funds.

5. QUOTE VALIDITY: Quote shall be valid for a minimum of 60 days after the Request for Quotation closing date.

6. REQUIRED DOCUMENTATION FOR AWARD: Vendor receiving the award is required to provide the following documentation before a purchase order is issued. Note: Do not purchase any materials until the approved purchase order is provided. No deposit will be provided.

(1) Hawaii Compliance Express Certificate of Vendor Compliance
(2) Certification of Liability Insurance (refer to Special Provisions Attachment 4)
(3) Signed Super Quote Award notice
(4) Listing of Joint Contractor and Subcontractor
(5) Itemization of quote upon request
(6) Performance bond if quote exceeds $50,000

7. QUESTIONS: shall be submitted via email to David Tamanaha, email davidt@hawaii.edu no later than 2:00 p.m. Hawaii Standard Time (HST), June 6, 2025.

8. Project shall be completed within 120 consecutive calendar days from the date designated in the approved Purchase Order.

9. SITE VISIT HIGHLY RECOMMENDED: To schedule a site visit, please contact David Tamanaha at 808-984-3253 or email davidt@hawaii.edu. The initial inspection is to identify and examine the conditions and to be aware or satisfied as to the character and amount of work to be performed as called for by the specifications. No additional allowance will be granted because of lack of knowledge of such conditions.

10. Inclusion of terms and conditions may be grounds for disqualification.

11. WARRANTY: The material furnished shall be new in accordance with the technical specifications. Used or refurbished goods will not be accepted. Contractor shall warrant that all goods furnished under this contract shall be guaranteed for the Manufacturer\'s Standard Warranty from the date of installation. Contractor shall replace and/or repair any defective goods at no cost to the University during the warranty period, provided such defects are not due to abuse or negligence on the part of the University.
