Special Instructions
Work to be performed at the NASA IRTF telescope located on the summit of Maunakea, elevation ~14,000 feet.

QUOTED PRICE MUST INCLUDE ALL COSTS for labor, materials, taxes, fees, shipping, handling, and delivery per line item.

Note: Due to the remote location of the delivery site, weather conditions could close the road when unsafe. Vendor to check the Maunakea Road Conditions by calling (808) 935-6268 or online at http://mkwc.ifa.hawaii.edu/current/road-conditions/. Delivery / work to resume when road conditions are safe.

Any award is subject to funding availability.

In accordance with the Federal Funding Accountability and Transparency Act (FFATA or Transparency Act), the RCUH must provide the Federal government with information regarding this procurement subcontract. Therefore, vendor must have a valid registration in the System for Award Management (SAM), the registration system for entities that conduct business with the federal government. FAR 52.203-11 Certification and Disclosure Regarding Payments to Influence Certain Federal Transactions and FAR 52.209-5 must be certified in vendor\'s registration. All bids are subject to the Research Corporation of the University of Hawaii (RCUH), Attachment 31-General terms and Conditions Applicable All Purchase Orders and Attachment 32B Federal Provisions - Government Subcontract Provisions Incorporated in All Subcontracts/Purchase Orders (Under Federal Prime Contracts). Attached are the terms for review and acceptance.

Each bid is subject to Section 103-53 of the Hawaii Revised Statutes, which requires a tax clearance from the Hawaii Department of Taxation and the U.S. Internal Revenue Service. Vendors are required to obtain the tax clearance prior to the execution of an agreement if $25,000 or more, and upon final payment. The awarded bidder shall execute an agreement with RCUH, which will state that RCUH Attachment 31 and 32B shall apply and will outline the insurance requirements.

The awarded bidder is required to provide RCUH with a certificate of insurance that satisfies RCUH.

Complete bid packet must include the following:
1) Bid for each line item, bidders must bid on all line items to be considered.
2) Copy of Current Certificate of Vendor Compliance issued by Hawaii Compliance Express.
3) Copy of Current Certificate of Insurance that satisfies RCUH requirements.
4) Recommended maintenance schedule and related costs.
5) Any quote must be valid for a minimum of 3 months from date of issuance.
6) Vendor\'s terms and conditions of sale.
7) Please quote each line item separately.
