Project Description:
1) To Supply and Replace Swing Door Operator on the Noi\'i Building, First Floor Main Entry ADA Right Door
Location: University of Hawaii Maui College, Noi\'i Building, First Floor Main Entry ADA Swing Operator Right Door Only Condition: Pair of Entry Doors with Right Door with Low Energy Swing Door Operator
2) Remove Existing Unit
3) Install new Swing Door Operator on the Right Door
Quantity: One (1) each: 8100 Record USA 8000 Swing Door Operator and Control, RHR, Surface Mounted, 36 inch Door Opening, Clear Aluminum Finish or approved substitution. Subject to prior approval if offering a substitute item.
4) Re-use existing power supply
5) Exit device must be unlocked during automatic operation.
6) Re-use existing card reader and push plate activation wiring.
7) Terminate, Program and Test to ensure proper functionality and safe operation
8) Installation shall be performed by a trained technician in accordance with Federal, State and County standards and applicable codes.
Price shall include all labor charges for removal of the old swing door operator, and installation of the new, programming and testing of new equipment, as well as the cost for materials, tools, equipment, shipping & handling, transportation, freight, travel expenses, and applicable taxes and fees.

2. Quote shall include all labor charges, as well as the cost for materials, tools, equipment, shipping & handling, transportation, freight, travel expenses, and applicable taxes and fees.
3. Quote must be valid for 60 days after the Request for Quotation closing date.
4. To be eligible to bid, vendors must possess a valid State of Hawaii Contractor\'s “C15” License
5. Questions & Requests for Substitution: shall be submitted to Melvin Hipolito, Jr., email: mh2350@hawaii.edu by 2:00 p.m. Hawaii Standard Time (HST), April 23, 2025.
If submitting a quote for a product substitute, advance approval is required. The University may, at its discretion reject or deny any product offer that it deems to not be equivalent, and the findings in this regard shall be final and binding.

7. Required Documentation for Award: Vendor receiving the award is required to provide the following documentation before a purchase order is issued. Note: Do not purchase any materials until the approved purchase order is provided.

(2) Certificate of Liability Insurance (refer to Special Provisions Attachment 4)
(3) Listing of Joint Contractor and/or Subcontractor (Form 54)
(3) Signed Super Quote Award notice
8. Project shall be completed within 120 consecutive calendar days from the date designated in the approved Purchase Order.
