Project Description: Replacement floor coverings at UH Maui College Hookipa Building Admissions and Records and Administrative Services.

Approximately 4,600 q. ft.

1. Furnish and install Karndean Commercial Vinyl Van Gogh luxury vinyl plank direct glue down flooring or approved equal. See Kardean Technical Data Sheet and manufacturer installation guidelines in Attachment #3.
2. Prep floor for installation over existing VCT (vinyl composition tile)
3. Remove existing cove base and install new 4\" rubber cove base throughout
4. Color to be determined for both flooring and cove base.
5. Furniture moving.
6. Contractor is required to remove and dispose of all waste, and shall not use the UHMC dumpsters.
7. See attached floorplans highlighting reflooring areas. Site visit highly recommended.
8. Moisture testing if required.
9. Door cutting as needed per room.
10. Supply and installation of threshold reducers at interior doors.

WARRANTY: The material furnished shall be new in accordance with the technical specifications. Used or refurbished goods will not be accepted. Contractor shall warrant that all goods furnished under this contract shall be guaranteed for the Manufacturer\'s Standard Warranty from the date of installation. Contractor shall replace and/or repair any defective goods at no cost to the University during the warranty period, provided such defects are not due to abuse or negligence on the part of the University.

2. REQUIRED DOCUMENTATION FOR AWARD: Vendor receiving award is required to provide the following documentation before purchase order is issued. NOTE: Do not purchase any materials until the approved purchase order is provided. No deposit will be provided.

1) Hawaii Compliance Express Certificate
2) Certificate of Liability Insurance & Workers\' Compensation (see Special Provisions & Documents Required)
3) Listing of Joint Contractor and Subcontractor
4) Signed SuperQUOTE award notice.
5) Itemization of quote upon request (see Sample - Attachment #5)
6) Performance and Payment Bond if applicable.

3. QUOTE VALID FOR: Minimum 60 consecutive calendar days after SuperQuote closing date.

4. CONTRACTOR LICENSE REQUIREMENT: C21 Flooring Contractor

5. QUOTE PRICE: Price shall be F.O.B. destination including all shipping, transportation and delivery charges, labor (including subcontractor labor) equipment and installation costs, warranty costs, expediting fees and all applicable fees and taxes (not to exceed 4.712% for Maui County).

6. BASIS FOR AWARD: Award will be based on the total aggregate amount. Award is subject to availability of funds.

7. DELIVERY PERIOD: To be scheduled as soon as possible after Purchase Order is awarded. Project shall be completed within 40 weekdays (Monday - Friday) from the date designated in the approved Purchase Order. Offices will be available for installation approximately between May to July 2025.

9. QUESTIONS and REQUESTS FOR SUBSTITUTION: shall be submitted via email to Melvin Hipolito, Jr. at mh2350@hawaii.edu no later than 3:00 p.m. Hawaii Standard Time April 21, 2025.

If submitting a quote for a substitute, advance approval is required. The University may, at its discretion reject or deny any product offer that it deems to not be equivalent, and the findings in this regard shall be final and binding.

10. PAYMENT: Contractor shall be paid upon final acceptance and submission of a properly executed original invoice and approved certified payrolls.
