Special Instructions
Location is the Komohana Research & Extension Center, 875 Komohana Street, Hilo, CA 96720.
1) Certificate of Insurance is required per example attached with University of Hawaii at above referenced site as certificate holder and additionally insured with waiver of subrogation for all polices listed and statement that insurance is primary and non-contributory. 2) Hawaii Tax Compliance certificate required.

4) Vendors who have not done business with the University of Hawaii, or have not done business with the University of Hawaii since July 1, 2012 must submit with this quote a recently dated and signed 2024 W-9 tax form per University policy.
5) It is the responsibility of all bidders to review the attachments to this Request for Quotation. Submission of a bid confirms a bidder has carefully examined the attached documents and agrees to the University of Hawaii\'s terms and conditions in place of Vendor\'s own terms and conditions.
6) Refer to attached mechanical plans for chiller system 7)Cost not to exceed $56,049.61. After award of this RFQ, if quoted labor hours end up being less and quotes for component costs and freight are less, vendor shall reduce invoice to reflect the reduction in costs relative to standard markup computed for estimated RFQ quote.
