2. Upon Request, the following information shall be provided to the University:
(1) Copy of product brochures for the listed equipment
(2) Warranty information
3. Hawaii Compliance Express: Quotes of $2,500 or more will require a Certificate of Vendor Compliance in compliant status at time of award (HRS 103D-310(c). If a vendor does not have a compliant certificate, the vendor\\\\\\\'s quote will be disqualified, and the next lowest quote will be considered. 4. Delivery schedule date: One hundred eighty (180) consecutive calendar days after receipt of the Approved Purchase Order from the University of Hawaii.
5. Quoted price: Prices shall be F.O.B. Destination including all shipping, handling, delivery and installation of new culinary equipment, and removal and disposal of the existing equipment and all applicable charges and taxes.
6. BASIS FOR AWARD: Award will be based on the total aggregate amount. Award is subject to availability of funds.

9. SITE VISIT is highly recommended. 10. Quote must be valid for 90 days after the Request for Quotation closing date.
11. Required Documentation for Award: Vendor receiving the award is required to provide the following documentation before a purchase order is issued. Note: Do not purchase any equipment until the approved purchase order is provided.

(2) Valid Certificate of Liability Insurance Certificate (sample of COLI attached for reference)
